The Shahada is the declaration of faith that makes a person Muslim. It is the first pillar of Islam.
أَشْهَدُ أَنْ لَا إِلَٰهَ إِلَّا اللَّهُ وَأَشْهَدُ أَنَّ مُحَمَّدًا رَسُولُ اللَّهِ
Ashhadu an la ilaha illallah, wa ashhadu anna Muhammadan rasulullah
I bear witness that there is no god but Allah, and I bear witness that Muhammad is His servant and messenger.

بِسْمِ اللَّهِ
Bismillah
In the name of Allah
الْحَمْدُ لِلَّهِ
Alhamdulillah
All praise is due to Allah
بِسْمِ اللَّهِ وَلَجْنَا، وَبِسْمِ اللَّهِ خَرَجْنَا، وَعَلَى اللَّهِ رَبِّنَا تَوَكَّلْنَا
Bismillahi walajna, wa bismillahi kharajna, wa 'ala Allahi rabbina tawakkalna
In the name of Allah we enter, in the name of Allah we leave, and upon Allah our Lord we rely.
بِاسْمِكَ اللَّهُمَّ أَمُوتُ وَأَحْيَا
Bismika Allahumma amutu wa ahya
In Your name, O Allah, I die and I live.
الْحَمْدُ لِلَّهِ الَّذِي أَحْيَانَا بَعْدَ مَا أَمَاتَنَا وَإِلَيْهِ النُّشُورُ
Alhamdulillahil-ladhi ahyana ba'da ma amatana wa ilayhin-nushur
All praise is to Allah who gave us life after death and unto Him is the resurrection.
